Please bring the following to every appointment:

•  your health card
• any glasses that you currently use
• a list of your medications
• any details of third party coverage you may have ( example Green Shield or Blue Cross )
• sunglasses in case you are dilated during the examination ( we have disposable sunglasses if you do not have any)
